If you are competing only, please complete pages 1 &2 of the application and submit along with your check or PayPal. Grand Champion and Reserve Grand Champion winners will be required to complete a W-9 form before receiving their prizes.
VENDORS
If you are competing and vending, or vending only, please complete pages 1 & 2 of the application and submit along with your Health Department Application, a copy of your vendor license, a completed W-9 form and your check.
